Washington Weight Loss and Family Practice
1. Initial New Patient Virtual Visit($200/visit)At this visit a virtual history and physical will be provided. Goals discussed regarding diet, exercise, weight loss and medication. Follow up planned. You will receive a follow up visit 7-10 days after visit to confirm your medication was received and tolerated.
2. Monthly Follow Up Virtual Visits($100/visit) At these visits a virtual history and physical will be provided. Progress regarding diet, exercise, weight loss, medication tolerance and dosage will be discussed. Refill and dosage adjustment for medications provided. Follow up planned. Monthly visits are needed until medication dose is titrated to goal. This usually take 1-6 months.Once titrated to goal dose of medications your virtual visits can be spaced further apart.
3. Weekly Coaching Program-"Transformation Seekers" available by Phone($80/month)
Phone calls are provided weekly to clients who enroll in our "Transformation Seekers" program from coaches who check in on how you are doing, offer support regarding diet and exercise.
Cancellation Fee-There is a $25 fee for all appointments cancelled with less than 24 hours’ notice.
Service is non-refundable.
